Intraoperative Management for Major Liver Resection: An Update

Patients coming for liver resection are increasingly comorbid and having increasingly aggressive resections. The rate of mortality has decreased substantially; however, the rate of postoperative morbidity continues to be high. Perioperative blood conservation techniques are a critical contributor to outcomes for patients having major liver resection. Receiving a blood transfusion during the surgical hospitalization is associated with poor outcomes. Recent large-scale randomized controlled trials have emerged regarding the use of intraoperative hypovolemic phlebotomy and antifibrinolytic medications (tranexamic acid) providing key insight to managing these patients. This session will aim to discuss an evidence-based approach to intraoperative care for patients having major liver resection and to delve into hypovolemic phlebotomy in detail.
After participating in this activity, participants will be able to:
- Identify evidence-based intraoperative management strategies for major liver resection surgeries.
- Consider implementing intraoperative hypovolemic phlebotomy for major liver resection surgeries.
- Articulate evidence-based perioperative blood conservation strategies for liver resection surgeries.
