Intraoperative Management for Major Liver Resection: An Update

Patients coming for liver resection are increasingly comorbid and having increasingly aggressive resections. The rate of mortality has decreased substantially; however, the rate of postoperative morbidity continues to be high. Perioperative blood conservation techniques are a critical contributor to outcomes for patients having major liver resection. Receiving a blood transfusion during the surgical hospitalization is associated with poor outcomes. Recent large-scale randomized controlled trials have emerged regarding the use of intraoperative hypovolemic phlebotomy and antifibrinolytic medications (tranexamic acid) providing key insight to managing these patients. This session will aim to discuss an evidence-based approach to intraoperative care for patients having major liver resection and to delve into hypovolemic phlebotomy in detail.  

After participating in this activity, participants will be able to: 

  1. Identify evidence-based intraoperative management strategies for major liver resection surgeries.
  2. Consider implementing intraoperative hypovolemic phlebotomy for major liver resection surgeries.
  3. Articulate evidence-based perioperative blood conservation strategies for liver resection surgeries.

Dr. McCluskey is known internationally for his clinical and academic interests in abdominal transplantation and perioperative blood conservation. He is a co-author on the HELIX trial and an anesthesiologist at Toronto General Hospital where he serves as the Anesthesia Director of Abdominal Transplantation and the Medical Director for the Surgical Blood Management Program.

Talk title: Intraoperative management for major liver resection: An update


Dr Ryan McGinn is an Anesthesiologist at The Ottawa Hospital. He completed his residency in Anesthesiology in Ottawa, followed by a fellowship in Abdominal Organ Transplant Anesthesia at Toronto General Hospital. Dr McGinn serves as the Clinical Lead for Kidney Transplant and Hepatobiliary Anesthesia. His research focuses on optimizing perioperative outcomes following major abdominal surgery.

Talk title: Hypovolemic phlebotomy in liver resection  

Dr Martel is a hepatobiliary and pancreas surgeon at The Ottawa Hospital. As the principal investigator for the PRICE-2 trial, he is known internationally for his work on perioperative blood conservation in liver surgery. He holds the Vered Family Chair in HPB Research and is a Senior Scientist at the Ottawa Hospital Research Institute.
